New feature: custom start and end pages

Besides the usual bug fixes (all outstanding bugs were resolved), there is a new feature available:

Custom start and end pages

This much asked feature is available on the options menu, and allows for the creation (via a wysiwyg editor) of a custom start page and end page (replacing the default one) for your surveys.
